ADMISSION AND DISCHARGE COORDINATOR – LPN/RN

FT : 11a-7p

This is a unique full time position dedicated to the admission and discharge of patients to our subacute program. You work 11a-7p, facilitating the smooth admission of patients to our Pleasant Bay sub-acute unit from various acute care settings. You also ensure that the hundreds of patients who go home each year after successful rehab stays are ready for success with the next transitions of care.

Recognizing that this schedule might not be the perfect fit for everyone, we will also consider applicants who can do some of the days, and offset full time as a day or evening shift staff nurse.

Are you unable to commit as a full time employee as a C.N.A.? Try our per diem program.

Per Diem I: The base pay rate for Per Diem I CNA is $14.00. This is a baseline per diem employee who is not “guaranteed” a certain number of shifts, and is not required to work a minimum number of shifts.

Per Diem II: The base pay rate for Per Diem II CNA is $15.00. This is an enhanced per diem employee, who accepts and meets a greater commitment level, based on facility need, which includes a minimum of four shifts per month, two of which must be on weekends.

Broad Reach Hospice and Palliative Care

RN Case Manager

Broad Reach Healthcare is hiring for a Full Time RN Case Manager position. 40 hours a week, with potential on-call shifts.

As our caseload continues to grow, we’re hoping to add compassionate, caring team players with hospice experience. The successful candidate will have strong assessment skills with a solid understanding of pain and symptom management, serving our patients both in the community and in several health care facilities o the mid and lower cape.

Weekend and On-Call Hospice RN Case Manager

Broad Reach Hospice is seeking a part time experienced hospice registered nurse to cover weekends. The work day will be from 8 am to 4 pm Saturday and Sunday and the On-Call portion will be from 4pm on Saturday to 8 am on Sunday and from 4 pm on Sunday to 8 am on Monday. Some flexibility of hours may be possible as well as additional hours during the week. The coverage area is from Barnstable to Eastham.  Attractive compensation and some benefits are offered.

Hospice CNA – Per Diem Shifts Available

Hospice is looking for a per diem CNA who is caring and compassionate, that will work under the direction of the RN Case Manager visiting patients in their homes, assisted living and skilled nursing facilities.

Broad Reach Hospice is a small friendly Hospice setting.

Must have CNA/HHA certification and a minimum of one year experience.

Hospice and/or home care experience preferred.

Weekend work is optional.

Victorian Assisted Living

Resident Service Assistant

The Victorian is seeking a full time (32 hours) resident service assistant (RSA) to work flexible hours to pick up other shifts as needed. HHA certification is desirable, but not required. In our assisted living residence, we serve non-memory care clients by helping residents enjoy life, by providing support in personal care, recreation, and assist with meal service and wellness checks. These important tasks become the foundation that lets staff establish outstanding relationships with some really great residents!

Full Time Cook

The Victorian is seeking a full time cook (30hrs) for Thursday and Friday from 11a-7p and every weekend from 7a-2p, plus opportunities to cross train and pick up more shifts.

Our assisted living at The Victorian offers gracious living in an intimate setting and provides a gourmet, full service, dining experience for approximately 40-50 individuals per meal.

If you want to work in a year round, stable, restaurant caliber environment but hate the usual hours this could be perfect for you. This is a full time position which offers full benefits, including paid health insurance, vacation time, holidays, sick/personal time, 401K plan, paid parental leave and competitive wage.

We are looking for a cook with restaurant or assisted living experience is preferred, but also who is creative, organized, flexible, able to work as a team, but also takes initiative and self-motivated. Strong sanitation skills are needed and ServSafe certification is also preferred but not a strict requirement.
